I got a text or email from my case manager

Your nurse case manager sent you a secure link to fill out or sign a form. Here's what to expect.

Last updated October 22, 2026

Who can do this: Patients

Steps

  1. Open the link in the text or email. It goes to casemgmt.io, the secure system your case manager's company uses.
  2. Confirm your date of birth (or the last 4 digits of your phone, for documents) so only you can open it.
  3. Fill in the form. To sign, draw your signature or type your name.
  4. Tap Submit Form or Sign & Submit. Your case manager gets the finished copy right away.

Can I reply to the text?

No — texts are one-way. Call or email your case manager directly. In an emergency, call 911.

How do I stop texts?

Reply STOP. Reply START to get them again, or HELP for help.

If something goes wrong

The link says it expired.
Form links work for 7 days and document links for 14 days. Ask your case manager to send a new one.
My date of birth isn't accepted.
Your case manager may have a different date on file. Call them to check it.
