Fill out or sign a form online

Step by step: open the secure link from your case manager, confirm it's you, fill in the form and sign it on your phone or computer.

Who can do this: Patients

Steps

  1. Tap the link in the text or email from your case manager.
  2. If asked, enter your Date of Birth (or, for some documents, the Last 4 digits of phone) and tap Continue. This makes sure only you can open it.
  3. Fill in the form. Fields with a red * must be filled in. If your case manager already filled it in, just check it.
  4. On a form, sign by tapping Draw signature and signing with your finger or mouse, or tap Type my name and tick the box that your typed name is your signature. Clear Signature lets you start over.
  5. On a document, type your Full Legal Name, draw your signature, and tick the box agreeing to sign electronically.
  6. Tap Submit Form or Sign & Submit. You'll see a thank-you message, and your case manager gets the finished copy.

If something goes wrong

"The date of birth does not match our records. Please try again."
Check the date and try again. If it still doesn't work, call your case manager; they may have a different date on file.
"Too many attempts. Please wait a few minutes and try again."
Wait a few minutes, then try again.
The link says it has expired.
Form links work for 7 days and document links for 14 days. Ask your case manager to send a new one.
"We can't verify your identity for this link. Please contact your case manager."
Your case manager needs to send the document again. Call or email them.
The document doesn't show on your phone.
Tap "Trouble viewing? Open the document in a new tab" under the document.
