Forms & e-signature

Filling out forms and sending forms and documents to patients to sign.

• Fill out a form and save it as a PDF

  Fill out a patient's form in CaseMgmt; their details fill in for you, and the finished PDF is filed in their Documents.

• Find your way around the Forms tab

  The Forms tab on a patient's file holds every form for that patient, sorted into five sub-tabs, plus the forms you sent out to sign.

• Get your own forms added

  Upload your own PDF forms yourself at no charge, or ask us to build a fill-in form for you as an optional paid service.

• Reuse a PDF from the Form Library

  Every PDF you upload for signature is saved to your company's Form Library, so you can add it to any patient with its fields already placed.

• Send a form to a patient by email or text

  The patient fills in and signs the form on their phone or computer, and the finished PDF is saved to their file.

• Start this month's report

  Use + New on a completed monthly report, progress report or office visit form to start the next one for the same patient.

• Upload your own PDF and send it for signature

  Upload any PDF, place the fields the patient must fill and sign, and send it by email or text; the signed copy files itself.
