Send a patient a secure link by text or email, and they fill in and sign the form on their phone or computer. The finished PDF is saved to the patient's file automatically. Your staff can also fill in forms themselves, with the patient's details already filled in.

Every form for a patient in one tab
The Forms tab on a patient's file holds intake forms, agreements, releases, client summaries and monthly reports, plus the forms you've sent out for signature and where each one stands.
Fill one in inside CaseMgmt and the patient's name, date of birth and other details fill in for you. The finished PDF is filed in their Documents. For a monthly or progress report, + New starts next month's from the last one.
Signatures without paper
- Send a form by email or text; the patient confirms it's them, fills it in and signs on any phone or computer.
- Upload any PDF of your own, place the fields the patient must fill in and sign, and send it the same way.
- Every PDF you upload is saved to your company's Form Library, with its fields placed, ready for the next patient.
- The signed copy files itself in the patient's documents.
- Texts go only to phone numbers that agreed to receive them.
Links that expire
Form links work for 7 days and document signing links for 14 days. If a link expires, send a new one in a few clicks.
Your own forms
Upload your own PDF forms yourself at no charge. If you'd like a fill-in form built for you, we offer that as an optional paid service.
Questions
Does the patient need an account?
No. The patient opens the secure link, confirms who they are, and fills in or signs the form. There's nothing to install.
Where does the signed form go?
The signed PDF is saved to the patient's Documents automatically, and the Forms tab shows it as completed.
Can we send forms by text?
Yes, to patients whose phone number has agreed to receive texts. Otherwise send it by email.
